PLEASE READ FOR IMPORTANT INFORMATION ABOUT THE EVENT!
When: Saturday 2/1812- (Sign-ups are limited and available at a first come first serve basis)
Who: Testing is for participants of Novato Lacrosse or other invited guests who are 10 YEARS & OLDER. Unfortunately, testing with this program is not available to athletes under the age of 10. As of 2/10/12, this clinic is available to other athletes in Marin. Please feel free to register if you are interested.
Where: Tamalpais High School - Computer Lab in Hoetger Hall. Hoetger Hall is up steps from the swimming pool. Park in main parking lot. (MAP)
Why: Baseline concussion testing provides a benchmark of which to compare future test results should an athlete experience a concussion injury. For more information, please visit www.sportconcussion.com or call (925) 988-0569.
Cost: $20 per participant + online registration fee (Cancellation notice must be received by 2/16 at 5 pm. or the payment will be non-refundable).As there is a cost to reserve the computer lab, notice of cancellation after the 2/16 deadline or no-shows will be non-refundable. Your understanding is appreciated.
Additional Information:
- Please arrive at least 5 minutes prior to your scheduled start time. As latecomers are dirsuptive to the testing process, individuals will have to wait until the next start time, space permitting. If you arrive late, you will not be permitted to test at your scheduled time! Your fee will not be refunded if you arrive late and are unable to test.
- Each participant MUST fill out the "Informed Consent" and the "Demographic form". These documents can be downloaded here.
- Registration will remain open up until 1 hour prior to the event starting. If there is space available, we will accept walk- ins. However, if we do not obtain a minimum number of participants then the entire event or selected time slots will either be cancelled or re-scheduled. This will be determined by 2/16 at 5 pm. You will be notified by email if the event is cancelled and will receive a full refund.
